Five tips for a smooth home health transition
Preparing a loved one — and your household — for in-home care.

Start with the care plan
Write down what help is needed, when, and who is responsible for what. Families often assume a shared understanding that turns out not to exist once care actually starts.
Include the things that are easy to forget: medication timing, dietary preferences, mobility limits, and how your relative prefers to be addressed.
Prepare the home
Clear routes between the bedroom, bathroom, and kitchen, improve lighting on stairs and landings, and move daily items to a height that does not require reaching or bending.
Set aside somewhere for care notes and supplies so nothing has to be hunted for mid-visit.
Give the relationship time
The first week or two is an adjustment for everyone. Routines settle, preferences become clear, and trust builds — usually faster than families expect.
If the fit genuinely is not right, say so early. Changing caregivers at week two is far easier than at week ten.
